ELEVAR Booking Contract In exchange for being permitted to enter the premises of ELEVAR, located at 1407 W 1st Ave, Spokane, WA 99201, I agree to the following as described in this contract. Customer Agrees to Terms and Conditions I hereby agree to comply with the Terms and Conditions of booking. I understand that I may be charged additional fees after my booking, based on the fees in the following terms: 1. Agree to leave ELEVAR and surrounding property in the same condition as when you arrived. Failure to do so will result in a minimum fee of $150. 2. You are responsible for resetting the space to its arrival condition. The entirety of your reservation includes you setting up and taking down. Please take a photo of the space and email it to brittani@elevarspokane.com - failure to do so will result in a minimum $150 fine. 3. You must obtain your own liability insurance. 4. We are not responsible for the personal property of renters or guests, including items left after leaving the premises. 5. It is renter's responsibility to clean the space, removing garbage and taking it with you, sweeping up any debris and wiping up any spills. Furniture must be returned to original location. A minimum fee of $150 will be charged for unsatisfactory cleaning or damage. 6. Refunds on cancellations will only be granted with 48-hour notice. Please email brittani@elevarspokane.com to cancel. 7. Please use CAUTION with all equipment and furniture on premises. You are liable for any damage caused during your rental time. Do not stand on chairs, sofas, or tables. 8. ELEVAR is a non-smoking environment, including e-cigarettes/vaping. No smoking inside of the studio at anytime. 9. Use of open flames (ex. candles) and/or pyrotechnics are strictly prohibited in the studio. 10. Please be courteous to other residents in the building, no loud music or excessive noise. 11. No pets allowed on premises. 12. All garbage needs to be taken with you when leaving the premises. -Customer releases seller from liability- I hereby expressly waive and release any and all claims, now known or hereafter known, against ELEVAR and its owners, employees, affiliates, and officers, on account of injury, illness, disability, or death arising out of or attributable to my being on the Premises and being exposed to or contracting COVID-19, whether arising out of the negligence of the Business, its owners, employees, affiliates, officers, or otherwise. ELEVAR has two cameras for security purposes ONLY. Your session/event will be kept confidential outside of behavior that is not in align with our agreement. The camera has a motion detection with no audio. You understand that all photography, filming and/or recording will be captured by these cameras. If you do not agree to these terms, please do not enter the event premises. This contract MUST be signed electronically. Electronic signatures count as original for all purposes.
