Terms of Agreement 1. $100.00 Deposit required to bind terms, deposits received are non-refundable. 2. Parties entering into this contract are permanently bound to fulfill the obligations by this agreement unless one or more of the following conditions apply: a. Both parties mutually agree to leave the agreement and its responsibilities. b. The event described herein is completely cancelled, and a minimum of 60 days’ notice is given. c. In the event that Jesse Galvan becomes unable to provide his services due to complications beyond his control, a reputable replacement shall be provided at no more than the original price agreed on. 3. Any and all information or details must be communicated before the 5 days preceding the performance. 4. Jesse Galvan shall not be held liable for any action arising from or in connection with any individuals not directly affiliated with Jesse Galvan. Furthermore, the party(s) contacting Jesse Galvan agrees to assume full responsibility for any and all damages caused by themselves or their guests involving any of Jesse Galvan’s property. 5. The parties purchasing the services of Rhythm Xpress agree to allow Jesse Galvan to use pictures and video images taken at this event for promotional purposes. The remaining balance will be due 7 days prior to the event date. Note: $25.00 fee and only cash or certified check will be accepted for the balance if paid on the day of the event.
